Advance Care Directives
An Advance Care Directive is a legal form that allows people over the age of 18 years to:
- make clear legal arrangements for your future medical and health care decisions
- write down your wishes, preferences and instructions for your future health decisions, end of life, living arrangements or personal matters
- appoint Substitute Decision-Makers to make informed decisions on their behalf when they are unable to do so themselves
Free consultations to assist you to put your Advance Care Directive together are being held at the Nuriootpa Library between 9:30am and 11:45am on various Saturday's throughout 2023.
This service is offered to our community by the Barossa, Gawler and Light Advance Care Planning Group and supported by The Barossa Council. All counsellors have completed Advance Care Directives training and have had police clearance checks.
